The EPC (Enhanced Primary Care) program, also known as Chronic Disease Management or CDM is a GP referral system that provides clients with chronic health conditions, a maximum of 5 allied health professional consultations under the medicare rebate within one calendar year.
What Is A Chronic Health Condition?
A chronic health condition typically lasts greater than 6 months and can include, but is not limited to any muscular, joint, neurological, balance, cardiac or respiratory condition. It should require a multi-professional team approach consisting of a GP and at least two other health care providers.
How Do I Know If I’m Eligible?
Eligibility is determined by the GP based on clinical judgement, the condition itself and personal needs so a management plan can be implemented.
Here at Core Physiotherapy & Pilates Studio, we accept the Medicare benefit as full payment for the service, so there will be no out-of-pocket cost to you.
The intention of the EPC program is to give people sessions they otherwise would not be able to afford to receive. These are services that assist their chronic condition and can include any of a number of our services.

Can I attend after my 5 sessions are over?
You can attend as a private client after your 5 free sessions. If you have private health insurance with extras you can use this. To find out more about our pricing, go to our ‘What Can I Expect To Pay’ page. If you are not with a health fund you can continue as a private client. To find out more about these fees please contact your nearest Core Physiotherapy and Pilates Studio near you. Also, if you have a concession card check out our info for Concession Card Holders.
