What is a Pessary?

A vaginal pessary is a removable silicone device inserted into the vagina to provide support to the pelvic organs, including the uterus, bladder, or rectum. Pelvic organ prolapse occurs when the supporting tissues (fascia and ligaments) weaken, allowing an organ to descend into the vaginal canal. A pessary sits high in the vagina to help support these structures and may reduce associated symptoms.

Pessaries do not correct or repair a prolapse but can lessen symptoms such as bulging, heaviness, or dragging sensations in the pelvis, and may assist with managing accidental urine leakage during activities like coughing, sneezing, or exercise.

Who Might Consider a Pessary?

Pessary Fitting Process at Core Physio Adelaide

Pessary fitting is performed by a qualified pelvic floor physiotherapist. During your appointment:

  • We trial different shapes and sizes to find the most suitable option for your anatomy and symptoms
  • Once inserted, you will be asked to move around, perform light activities, and attempt to empty your bladder to check comfort and retention
  • You will be taught how to insert, remove, and care for the pessary yourself (most women learn this quickly)
  • A follow-up review is usually scheduled within 2 weeks, then every few months as needed


Some pessaries can remain in place during intercourse, while others need removal – your physiotherapist will advise based on the type selected.

Ongoing Pessary Care and Review

Regular care helps maintain vaginal health and pessary function:

  • Many pessaries can be removed nightly or weekly (as advised) for cleaning with warm water and soap-free cleanser
  • Vaginal oestrogen cream (prescribed by your GP if appropriate) may be recommended, especially after menopause, to support tissue health
  • Review appointments with your physiotherapist every 3–6 months for professional cleaning, vaginal examination, and pessary replacement (usually yearly)

Possible Side Effects and What to Watch For

Like any medical device, a pessary may sometimes cause vaginal irritation, increased discharge, or minor discomfort. These issues can often be managed by temporary removal, refitting a different size/shape, or using prescribed vaginal oestrogen. Seek prompt medical advice if you notice:

  • Unusual vaginal bleeding or offensive odour
  • Persistent discharge
  • Difficulty emptying your bladder
  • Signs of infection

Treatment and Sessions

Your physiotherapist will review your medical history, perform an internal assessment, and fit the pessary. You’ll learn how to insert, remove, and clean it, and receive advice on pelvic floor exercises to complement the treatment.
